Course Learning Outcomes

1. Describe theoretical models and frameworks of health promotion for individuals with chronic illness and disability.
2. Compare and contrast exemplary health promotion efforts for individuals with chronic illness and disability, focusing on the theoretical framework, methods of intervention and evaluation design used.
3. Engage in dialogue across professional disciplines related to health and disability.
4. Identify health issues, health promotion and health practices that increase functioning and reduce secondary conditions due to disability and chronic illness.
5. Demonstrate the health promotion process including community assessment, resource identification, intervention strategies and evaluation using state-of-the-art approaches.
6. Adapt health promotion strategies to individuals with chronic illness and disability, including those from ethnic minority and special population groups.
7. Foster positive, productive relationships with all stakeholders that allow for effective collaboration.
